Appearance needs acquisition context

Link an observation to structure, acquisition conditions and the source’s reporting language. A pattern detached from context cannot carry a conclusion.

Organise retrieval by anatomy, relation and source question. Preserve the difference between a revision concept and a complete diagnostic assessment.
